Hi everyone! Welcome to the 2026-2027 school year. There are important papers on the school district website that we ask you to fill out and bring into the main office as soon as possible. The S865 is permission for your child to receive tylenol/motrin while in school. This paper also covers any emergency medicine they may need.

The Physical exam form (MEH-1) MUST be filled out by a PHYSICIAN or PROVIDER.

The Med-1 Form is paper that must be filled out by a PROVIDER for any prescription medications your child needs to receive during the school day.

As always, when your child goes to the doctor, we encourage you to request a physical exam paper as well as immunization records.
